On a full retrofit it is likely that all the devices will be replaced. It is likely that devices will be added on a full retrofit because the AHJ might require update to current code.
However, on a service call to replace one A/V that failed during inspection, it will be just that device replaced. 4-wire device replaced with 4-wire device is the quickest and easiest. 4-wire devices are still needed.
