What is the worst fire alarm fail you have ever seen?

At least it’s not a code violation being the strobes can be mounted at any angle and there wasn’t any fire lettering to make a violation.

This Honeywell BG-12 pull station isn’t easilly accessible to pull, also the SpectrAlert Classic is also kinea obscured.

Saw a System Sensor L-series outside on a standard double gang back box.

A resort that I stayed at in Hawaii last month had a HUGE Simplex voice evac system (most likely with multiple nodes). Being Hawaii, there were lots of outdoor spaces. All of the outdoor spaces had notification appliances but I don’t think I saw a single outdoor rated one in the whole resort. For example, I saw many devices ranging from the 80s-today that are not outdoor rated in the outdoor lobby. I even saw a smashed and very weathered TrueAlert speaker strobe. Sadly, I don’t have any pictures.

I also saw an EST Genesis speaker hanging by the wires off the wall in my hotel room in Colorado.

My former high school still has its original fire alarm system, a few fails I found were some of the 4050 horns were falling off the back boxes and one set of signaling devices were painted over. That building also has many code violations, such as outdated fire doors with disconnected closer mechanisms and two sets of fire doors that are non-existent. I have a post on the “what fire alarm do you have at work or school” category. I have since transferred to a more up-to-date public high school in the same city because that building gave me a ton of sinus infections due to asbestos everywhere and the rear part of the building resembles the Backrooms. I will be glad if that building is demolished in a few years from now.

It looks like the whole LCD screen is messed up.

I saw one at my school today. One of the Simplex Truealert speaker strobes in the gym had the cage starting to fall off.

I saw something like that at the national zoo.

Kind of a old one that I saw before I had a cellphone, but I’ve seen once a Esser IQ8 smoke detector covered with an entire roll of gaffer tape.

I’ve seen a few, like a store that had a old Chubb/Proteg system on which the panel was showing a litany of battery/power troubles, but that gaffer-covered IQ8, that one takes the cake IMHO.
