The 4045-13? I think Dan M.'s middle/high school had those in several areas, except they were the non-fire version, single-stroke and were flush-mounted behind grilles. (The actual alarms were originally older 10-inch Simplex bells, now they are newer Simplex bell/strobes.) A couple of the chimes were replaced with bells so they could be louder.
